New Members of Northside Bible Academy
Fill out the following forms:
- Church School Enrollment Form (One for each child). Be sure to sign in parts I & III.
- Northside Bible Academy Family Enrollment Form.
- Northside Bible Academy Student Form (one for each child).
- Signed and notarized Waiver of Liability Form.
Submit Annual Membership fee of $50/family with one child or $75/family with two plus children. A one-time admission fee of $25 will be charged the first year.
Returning Members of Northside Bible Academy
You do not need to fill out a Church School Enrollment Form or Waiver of Liability Form unless you have moved in the last year or unless you have a child who is turning seven this year. If so, then complete a CSEF for each child (if moved) or complete a CSEF for the child who is turning seven this year.
Submit an updated Northside Bible Academy Renewal Form for our records by the end of the first week of August.
Send appropriate re-enrollment fee of $50/family with one child or $75/family with two plus children.
